Surgical treatment of odontoid fractures with C1 hook and C2 pedicle screw construct.

چکیده
A number of approaches have been attempted to address the treatment of acute odontoid fractures, spanning from conservative treatment with an external orthotic device to posterior C1-C2 arthrodesis or anterior screw fixation. The current report describes stabilisation of such fractures with a C1 supralaminar hook and a C2 pars interarticularis screw construct. Two patients with odontoid fracture were treated by this technique. Two-year follow-up of both patients revealed complete union without complications. The authors stress that this technique avoids the hazards of C1 lateral mass screws.

C1-c2 pedicle screw fixation for treatment of old odontoid fractures.
Nonunion and C1-C2 instability of odontoid fractures usually result from delayed diagnosis and inappropriate treatment. However, the available treatment options for odontoid fractures remain controversial. The authors evaluated the effectiveness of internal screw fixation via the C1 and C2 pedicle in cases of old odontoid fractures. This retrospective study included 21 patients with old odontoi...
متن کاملBiomechanical evaluation of four different posterior screw and rod fixation techniques for the treatment of the odontoid fractures.
Problems that screw cannot be inserted may occur in screw-rod fixation techniques such as Harms technique. We compared the biomechanical stability imparted to the C-2 vertebrae by four designed posterior screw and rod fixation techniques for the management of odontoid fractures. A three-dimensional finite element model of the odontoid fracture was established by subtracting several unit structu...
متن کاملTreatment for odontoid fracture with C1 lateral mass and C2 pedicle screws using intraoperative orbic-3-dimensional navigation
Objective: To assess the technical advantages of intraoperative orbic-3-dimensional navigation in the treatment of odontoid fractures. Material and methods: Twelve male patients with type II or rostral type III odontoid fractures were treated with posterior C1LM-C2P screws fixation using the orbic-3D fluoroscopic navigation. The follow-up period ranged from 12 to 24 months with a mean of 19.2 m...
متن کامل8 Microsurgical Treatment of Odontoid Fractures
The first two cervical vertebrae, C1 and C2, are also known as the atlas and axis, respectively. The odontoid process of C2, also known as the dens, is the superiorly projecting bony prominence from the body of C2 that sits within the anterior portion of the ring of C2. C1 rotates around this. Anterior odontoid screw fixation involves the reduction and stabilization of an odontoid fracture usin...
متن کاملTreatment modality in type II odontoid fractures defines the outcome in elderly patients
BACKGROUND Odontoid fractures account for approximately 20% of all fractures of the cervical spine. They represent the most common cervical spine injury for patients older than 70 years, the majority being type II fractures (65-74%), which are considered to be relatively unstable. The management of these fractures is controversial. Possible treatment options are either conservative or surgical....
